  • Amount consumed per day
  • Current age
  • Any diagnosis of abuse or dependence
  • Treatment, if any (Alcoholics Anonymous, medication, medical monitoring, etc.)
  • History of relapse, if any
  • Any medical complications
  • Co-morbid conditions
  • Any history of drug use or mental illness
  • Any change in drinking habits and reason for the change
  • Member of a self-help group
  • Ever had a DUI
  • Ever been advised to reduce consumption or seek treatment
