Sonata Health policy
provides unique drug support.

Great-West Life‘s individual health and dental policy is called Sonata Health. The plan comes with five plan design levels, including a guaranteed acceptance plan. Each plan level provides varying levels of coverage. The insured can also add up to four optional benefits to the plan, including the following:

1. Coverage for hospital accommodations.

2. Emergency travel.

3. Cash benefits while the insured is in hospital.

4. Accidental death and dismemberment benefits.

In addition, all Sonata Health policies have the following built-in supplemental benefits:

1. Quick claim benefits, including an option to sign up for direct deposit.

2. An Assure card, which is recognized by more than 7,500 pharmacies nationwide. Using the Assure card makes using the insured’s prescription drug coverage extremely easy.

3.  A Direct Deposit option allows payments be deposited directly to the insured’s bank, free of charge.

4.  The PVS Vision Care and hearing aid program provides a discount on quality eyewear and hearing aids.

    I have your coverage for dental through Queen’s University. Have expense claim for visit to dentist on December 6, 2012 and would like to send the dental form filled out by dentist. It is a standard dental form not Queen’s University’s but I have the Queen’s policy number. What should I do?
